About The Position

The Clinical Manager Therapist is a licensed Physical or Occupational Therapist who oversees a team of clinicians in all day-to-day of patient care including, but not limited to: regulatory and billing compliance, scheduling of adequate staff, Interdisciplinary Team (IDT) meetings and utilization, customer service and provision of medication, services, equipment and supplies in accordance with the individualized plan of care and developed by the patient, physician and interdisciplinary team.

Requirements

  • Licensed Physical or Occupational Therapist
  • Possesses and maintains current CPR certification (BLS required).
  • Possesses and maintains a valid driver’s license licensed driver.
  • Possesses and maintains an automobile that is insured in accordance with LORIAN requirements.
  • Knowledge of Microsoft Office Systems.
  • Working knowledge of State regulations (e.g. Title 22 (CA), or Accreditation Commission for Health Care, and Federal Medicare Conditions of Participation regulations.
  • Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
  • Has the Ability to read and follow written instructions and document care given in the patients’ electronic medical record.
  • Is flexible and cooperative in fulfilling the role obligations.
  • Knowledge and ability to apply community health principles and practices.
  • Ability to supervise, guide, and develop skills and performance of service personnel.
  • Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

Responsibilities

  • Oversees and ensure delivery of all patient care and services according to professionally-accept standards of practice.
  • Participates with the physician, referral sources and professional staff in decisions regarding patient eligibility and suitability for home health services
  • Ensures that upon admission to LORIAN all patients are thoroughly assessed and that based on that assessment the appropriate clinical services are rendered to the patient.
  • Coordinates regular case/team conferences and promotes communication of information relevant to the patient care process to ensure quality of care and coordination of services.
  • Assesses clinical practices of specified disciplines through the review of electronic clinical records to ensure that standards of care are met and quality services are provided in accordance with prescribed LORIAN’s, local, state, and federal guidelines.
  • Coordinates staffing patterns to optimize utilization of clinical staff while at the same time meeting patient needs and the needs of the interdisciplinary geographical clinical teams.
  • Reviews intervention by different disciplines and minimizes duplication of effort and overlapping of services (i.e. exhaust OT intervention prior to referring HHA).
  • Reviews patient’s address and assigns patients to clinicians respective of their territory.
  • Collect and analyze staff visit data to ensure that individual clinicians consistently meet prescribed productivity standards.
  • Collaborate with Quality Assurance Department in the dissemination and implementation of new regulatory and clinical guidelines.
  • Completes, maintains, and submits accurate and relevant clinical notes regarding patients/clients’ condition and care given in accordance with LORIAN policies and procedures.
  • Participates in in-service education programs and clinical meetings.
  • Protects patients and employees by adhering to infection-control policies and protocols.
  • Maintains confidential patient records in accordance with privacy and security standards of the Health Insurance Portability and Accountability Act (HIPAA) and other applicable state laws.
  • Comply with accepted Professional standards & practices.
  • Performs other related duties incidental to the work described herein.
  • Involved with hiring, leading and developing team members to support Lorian’s mission.
  • Act as preceptor to direct reports.
  • Act as mentor to direct reports.
  • May be required to perform field visits as directed by DPCS.
  • May be required to participate in after hours and weekend on call rotation.
