The Therapist provides outpatient mental health assessments and counseling, along with provider education and consultation, through the CREATE (Co-designing Rich Environments for All to Thrive Everyday) program. Services may be delivered in clients’ homes, an office, or via telehealth. The program serves Duval County children and their parents, guardians, and caregivers who are referred for emotional, behavioral, or developmental concerns, including families with complex medical needs; youth transitioning from pediatric to adult care (HCT); and LGBTQ+ children and youth. The goal is to stabilize families, parent-child relationships, and mental health, or to prevent entry into formal systems of care. The role requires a master’s degree in Social Work or Mental Health from an accredited college or university and eligibility to become a registered intern with the State of Florida. One year of counseling experience and licensure are preferred. Registered interns must meet Florida’s training requirements and arrange weekly clinical supervision. The Therapist documents counseling sessions and completes required program reports, participates in staff and program meetings, consults with licensed staff and the Project CREATE Director about appropriate services, and identifies training needs with the Director. Additional duties may be assigned. Employment is contingent on required background, reference, drug, motor vehicle, insurance, education-verification, and E-Verify checks.
