About The Position

Help make behavioral health care accessible when clients need it most. This role provides individual and group therapy to individuals living with serious and persistent mental health conditions and co-occurring chronic physical health conditions that significantly affect their daily functioning and ability to access care. The therapist will meet clients where they are, which may include seeing them in the office, through telehealth, or in their home or another community setting. The goal is to help clients build greater independence, reconnect with their communities, and access treatment in the least restrictive setting appropriate for them. This is a clinical role for someone who believes effective behavioral health treatment means meeting clients where they are. The Integrated Care program uses a flexible hybrid and community-based model, with a typical week including office, community/home visits, and telehealth services. The schedule is flexible and can change based on client needs.
