About The Position

Mindoula Health is seeking a Therapist (LCSW, LPC, LMFT) licensed in Tennessee to join our Substance Use Disorder Program and serve our substance-exposed members. This position is mostly work from home, but will require some travel 25% of the time when needed. Our Substance Use Disorder Program focuses on prenatal and postpartum care, as well as supporting individuals facing substance-exposed living. As part of this program, you’ll work in a family-focused culture, gaining valuable experience in both the clinical and business sides of the company. Requirements

  • LCSW, LPC, LMFT in Tennessee.
  • Experience with adults and adolescents.

Nice To Haves

  • Preferred experience with substance abuse population.
  • Background in maternal; substance abuse preferred.
  • Familiarity with Medicare and Medicaid procedures.
  • Remote Work Experience.

Responsibilities

  • Assess, plan, and implement care strategies that are individualized by members and directed toward the most appropriate and least restrictive level of care.
  • Collaborate with the member, family, and healthcare providers to develop an individualized plan of care.
  • Conduct individual counseling and/or group therapy with adolescents and adults.
  • In person/field work as needed for families with TN DCS involvement.
  • Identify and initiate referrals for social service programs – including financial, psycho-social, community, and state supportive services.
  • Advocate for members and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
  • Utilize approved clinical criteria to assess and determine the appropriate level of care.
  • Document all member assessments, care plans, and referrals provided.
  • Achieve set goals and Key Performance Indicators (KPIs).
  • Learn the Substance Use Disorder Program model and take responsibility and ownership for outcome-based care.
  • Participate in interdisciplinary team meetings and utilization management rounds and provide information to assist with a safe transition of care.
  • Promote responsible and ethical stewardship of company resources.
  • Maintain excellent punctuality and attendance during work hours.
  • Perform other duties as assigned.
