Requirements

  • Master’s Degree in Social Work, Counseling, Psychology, or related behavioral health field
  • Current CPR & First Aid Certification
  • Current TB Test
  • Fingerprint Clearance Card

Nice To Haves

  • Proven experience in home & community care settings or related behavioral health environments.
  • Strong background in data collection, patient care, medical terminology, and medical documentation practices.
  • Expertise in addiction counseling, social work principles, behavioral therapy modalities, and working with children or individuals with disabilities.
  • Familiarity with diagnostic evaluation processes and mental health assessments aligned with HIPAA standards.
  • Knowledge of applied behavior analysis (ABA), crisis intervention techniques, and case management strategies.
  • Experience working with individuals with autism or developmental disabilities is highly desirable.
  • Excellent communication skills for individual / group counseling sessions and conflict management situations.
  • 2+ years of experience in behavioral health, addiction treatment, or mental health services
  • Experience facilitating therapy groups and treatment planning
  • Active LMSW license
  • Knowledge of mental health and substance abuse treatment
  • Strong interpersonal, communication, and organizational skills
  • Ability to work effectively in high-stress or crisis situations

Responsibilities

  • Conduct thorough patient assessments to identify mental health needs, including diagnostic evaluations and intake procedures.
  • Develop and implement individualized treatment plans utilizing therapies such as cognitive behavioral therapy, motivational interviewing, art therapy, and applied behavior analysis.
  • Provide both individual and group counseling sessions focused on mental health counseling, addiction counseling, child & family counseling, gender issues counseling, and crisis intervention.
  • Collect and maintain accurate medical documentation in compliance with HIPAA regulations, ensuring confidentiality of medical records and records management.
  • Engage in case management activities to coordinate care with other healthcare providers, social workers, educators, and community resources.
  • Utilize behavioral health techniques to manage behavior issues, conflict resolution, crisis management, and behavior management strategies.
  • Work with individuals with developmental disabilities or autism spectrum disorder (ASD), including those requiring special education or PTSD care support.
  • Conduct psychosocial assessments and develop diagnostic impressions using DSM criteria.
  • Complete progress notes, treatment plans, discharge summaries, and clinical documentation accurately and on time.
  • Maintain compliance with licensing, ethical, and agency standards.
  • Participate in clinical staffing and treatment team collaboration.
  • Coordinate care with community resources and referral partners.
  • Work closely with interdisciplinary treatment teams.
  • Participate in after-hours, weekend, or holiday coverage rotation when assigned.
  • Create individualized treatment plans in collaboration with clients and families.
  • Provide individual, group, and family counseling sessions.
  • Facilitate psychoeducational and support groups.
  • Provide crisis intervention services as needed.
