About The Position
Quality Moments is seeking a compassionate, client-centered and reflective professional to join our Clinical Team and provide high-quality behavioral health services to adults within community and long-term care settings. Requirements
- Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or another equivalently licensed mental health professional required.
- A strong desire and commitment to improving the quality of life for aging adults.
- Prior experience with individuals experiencing out of home placement, the aging population, or other vulnerable populations.
- Strong interpersonal and communication abilities, including documentation, presentation, and real-time problem solving.
- Valid driver’s license and reliable transportation with current automobile insurance meeting state minimum liability requirements.
Responsibilities
- Provide high-quality behavioral health services to adults within community and long-term care settings.
- Conduct assessment, treatment plan development, therapeutic interventions, and case management services.
- Maintain professional ethical standards and adhere to all confidentiality and privacy requirements, including HIPAA.
- Document services provided.
- Travel between surrounding areas to work with clients in the community.
