About The Position
ViaQuest Psychiatric & Behavioral Solutions offers unique and individualized care to adults who reside in long-term care facilities. Our clients are supported by a team of Certified Nurse Practitioners and Mental Health Counselors and Social Workers to ensure the best outcomes for those we serve. This role involves performing comprehensive diagnostic assessments, providing patient visits, ensuring treatment plans are up-to-date, completing progress notes, communicating client status, and collaborating with case management and psychiatric services.
Requirements
- Master’s degree in social work or professional counseling
- Social Work/Professional Counseling License or equivalent: LCSW or LMHC
- Valid driver's license, car insurance and reliable transportation.
- Willingness to travel throughout assigned service area.
Nice To Haves
- 2 years’ experience in therapy and counseling is preferred.
Responsibilities
- Performing comprehensive diagnostic assessments.
- Providing patient visits as scheduled.
- Ensuring treatment plans are up to date for each client.
- Completing comprehensive and succinct progress notes.
- Communicating and following up as needed regarding the client's status.
- Working collaboratively with those providing case management and/or psychiatric services.
