About The Position

This role serves as a program leader within the Clay Outpatient setting, focusing on clinical responsibilities, personnel administration, supervision, and record keeping. The Therapist Team Leader is responsible for providing comprehensive clinical assessments, developing treatment plans, and delivering individual, group, and family therapy. The position also involves leadership duties such as training staff, conducting performance evaluations, and ensuring staff competence. Maintaining accurate and timely clinical documentation is a key aspect of this role.

Requirements

  • Master’s degree in a mental health discipline.

Responsibilities

  • Maintains an active role as program leader.
  • Provides effective clinical assessment of the consumer.
  • Utilizes assessment data to support a DSM V diagnosis.
  • Formulates appropriate treatment plan along with consumer and families, as appropriate.
  • Provides individual, group and family therapy, utilizing basic therapeutic models and theories according to consumer needs as stated in the treatment plan.
  • Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation.
  • Effectively collaborates with the treatment team members and outside sources in the delivery of clinical services.
  • Provides support and education for consumers and family members.
  • Evaluates the consumer for discharge planning.
  • Facilitates services that prevent commitment.
  • Adheres to professional code of ethics.
  • Effectively demonstrates the application of adult and child/adolescent specific competencies.
  • Assists with seclusion and restraint per AltaPointe policy and procedures.
  • Formulates a person centered, recovery oriented, medically appropriate treatment plan along with consumer that proves medical necessity and obtains signature when plan is created, updated and/or revised.
  • Assists supervisors with interviews when necessary and completes appropriate forms as outlined by Personnel Policy and Procedures.
  • Provides training in the general orientation of the facility, policies and procedures, program operations and specific skill areas.
  • Is knowledgeable and applies personnel policies and procedures on a consistent basis.
  • Objectively completes employee performance evaluations on supervised staff and routes to Personnel Department on a timely basis.
  • Provides positive and effective leadership.
  • Provides staff with consultation and supervision as needed, individually or through group staff meetings.
  • Ensures competence of staff.
  • Monitors staff work activities to ensure that they are performing all duties as assigned.
  • Seeks clinical supervision and consultation as needed.
  • Accepts and employs suggestions for improvement.
  • Actively works to enhance clinical skills.
  • Documents in a timely fashion per AltaPointe policy.
  • Documents appropriate, complete, and clinically comprehensive mental health assessments.
  • Documents measurable and specific problems, goals, and objectives in the treatment plan.
  • Documents in a clear and concise manner, types of consumer problems, treatment goals and objectives addressed, services, and techniques used, consumer response and progress and plan for continued services.
  • Documents case disposition, case closings, follow-up and referrals.
  • Documents legibly and provides credentials with each signature.
  • Learns and utilizes Avatar.
  • Documents incidents through RL-6.
