About The Position

Crossroads Health, a 501(c)(3) non-profit organization with facilities located in Mentor, Painesville, and Cleveland, serves Northeast Ohio communities with comprehensive behavioral and primary integrated healthcare, early childhood services, extended housing and recovery services—no matter one’s ability to pay. Our mission is to provide hope, healing, and healthcare to everyone. We seek an Therapist for our Pepper Pike, OH facility. This Therapist is responsible for coordinating communication and services among clients and interested parties to ensure seamless service delivery consistent with the client treatment plan. The Therapist works not only with adolescents, but the families, legal guardians and other interested parties to create and implement a treatment program.

Requirements

  • Associate’s degree required
  • CDCA certification required.
  • Experience working with adolescents and their families involving addiction and mental health treatment
  • Experience recognizing the various socio-economic and cultural factors within which addiction and substance abuse exist and how to overcome obstacles to sobriety.
  • Experience working with parents and legal guardians of the clients in a family counseling setting.
  • Must have valid driver's license and an acceptable driving record (4 pts. or less), reliable transportation, and carry automobile insurance with minimum liability limits of $100,000/$300,000 and property damage of $50,000.

Nice To Haves

  • Bachelor’s degree preferred.
  • LSW or LPC preferred.

Responsibilities

  • Coordinating and scheduling services for clients/families.
  • Providing updates to family members, internal/external service providers, referral sources, and others (probation officers, etc.) as each case requires.
  • Facilitating/co-facilitating group sessions.
  • Providing practical skills focused individual therapy.
  • Makes referrals for clients and/or family members as appropriate.
  • Identifies potential areas for crisis and proactive plans to prevent it.
