About The Position

Beyond Faith Homecare & Rehab is seeking compassionate and dedicated therapy professionals, including Physical Therapists (PTs), Occupational Therapists (OTs), and Speech-Language Pathologists (SLPs), to join our home care team. Requirements

  • 1 year experience as a therapist in a home health setting.
  • High School Graduate or equivalent
  • Current CPR certification.
  • Current driver’s license in good standing.
  • Reliable transportation and auto liability insurance.
  • On-going employment as an HHA always requires the maintenance of a valid HHA certificate in the state where the caregiver practices and a valid CPR certificate.
  • Prolonged standing/walking is required.
  • The ability to lift/transfer up to 50 lbs.
  • Requires working under stressful conditions to meet deadlines and patient needs and make quick decisions and resource acquisition.
  • Meet patient/family individualized psychosocial needs.
  • Requires hand-eye coordination and manual dexterity.

Nice To Haves

  • Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules.

Responsibilities

  • Provides skilled therapy services to patients in their place of residence in accordance with the physician's plan of care and professional standards of practice.
  • Evaluates, develops, and implements individualized treatment plans designed to maximize each patient's functional abilities, independence, safety, and overall quality of life.
  • Collaborates with physicians, caregivers, and the interdisciplinary care team to monitor progress, educate patients and families, and adjust treatment plans as needed.
  • Maintains accurate and timely clinical documentation.
  • Assist clients with tasks within the scope of job description based on the Plan of Care (POC).
  • Communicate effectively with administrative staff regarding client/family needs assessments, including reporting any significant changes in client condition.
  • Provide care as outlined in the Care Plan and accurately document the cares provided.
  • Contribute to continuity of care, effective communication between all care team members, and regular participation in Case conferences/Team Meetings.
  • Apply concepts of infection control and universal precaution in coordination/performing client care activities to protect both staff and patients.
  • Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.
  • Completes training or in-services in a timely manner as assigned by your supervisor.
  • Meet mandatory continuing education requirements of the Agency/licensing board.
