About The Position

Beyond Faith Homecare & Rehab is seeking compassionate and dedicated therapy professionals, including Physical Therapists (PTs), Occupational Therapists (OTs), and Speech-Language Pathologists (SLPs), for current and future opportunities in home care. This is a general application for licensed therapists passionate about delivering high-quality, patient-centered care in the home. The role involves helping patients improve their independence, mobility, communication, and overall quality of life. The therapist will provide skilled therapy services in patients' residences according to the physician's plan of care, evaluating and implementing individualized treatment plans to maximize functional abilities, independence, safety, and quality of life. Collaboration with physicians, caregivers, and the interdisciplinary care team is essential for monitoring progress, educating patients and families, and adjusting treatment plans, all while maintaining accurate and timely clinical documentation. The position involves interaction with patients of all ages and requires understanding and demonstrating appropriate behavior when interacting with diverse patient populations.

Requirements

  • 1 year experience as a therapist in a home health setting.
  • High School Graduate or equivalent.
  • Current CPR certification.
  • Current driver’s license in good standing.
  • Reliable transportation and auto liability insurance.
  • Ongoing employment as an HHA always requires the maintenance of a valid HHA certificate in the state where the caregiver practices and a valid CPR certificate.
  • Prolonged standing/walking is required.
  • The ability to lift/transfer up to 50 lbs.
  • Requires working under stressful conditions to meet deadlines and patient needs and make quick decisions and resource acquisition.
  • Meet patient/family individualized psychosocial needs.
  • Requires hand-eye coordination and manual dexterity.

Nice To Haves

  • Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules.

Responsibilities

  • Assist clients with tasks within the scope of job description based on the Plan of Care (POC).
  • Communicate effectively with administrative staff regarding client/family needs assessments, including reporting any significant changes in client condition.
  • Provide care as outlined in the Care Plan and accurately document the cares provided.
  • Contribute to continuity of care, effective communication between all care team members, and regular participation in Case conferences/Team Meetings.
  • Apply concepts of infection control and universal precaution in coordination/performing client care activities to protect both staff and patients.
  • Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency.
  • Completes training or in-services in a timely manner as assigned by your supervisor.
  • Meet mandatory continuing education requirements of the Agency/licensing board.
