  • Bachelors/Master's degree in counseling, marriage and family therapy, psychology or similar degree (dependent on program)
  • One or more years of experience providing direct care
  • Must have and maintain a valid driver's license with an acceptable driving record
  • Must be able to exercise independent thinking and good judgment under all circumstances.
  • Must be able to analyze work, set goals, develop plans, and utilize time effectively and efficiently.

Responsibilities

  • May provide counseling including relationship, premarital, separation, divorce, and child counseling services
  • Gathers and analyzes information through a variety of methods that may include observation, interviews, self-report inventories, group discussions, and/or formal assessments
  • Identifies problematic behavioral, communication, and relationship patterns
  • Partner with clients to monitor adherence to their treatment plan, evaluate effectiveness, monitor progress in a timely manner, and facilitate changes as required
  • Facilitate client linkages, referrals and access to appropriate treatment, community supports and medical or specialty providers according to the treatment plan
  • Maintain an active caseload
  • Develops and maintains accurate records, reports and chart documentation in a complete and timely manner according to program guidelines and established procedures
  • Ensures quality and accuracy of written and verbal communications
  • Support the Quality/Compliance/Risk function as required
  • Drafts and submits necessary paperwork to insurance companies
  • Maintains compliance with ethical codes and standards
  • Performs other related duties as assigned
  • Establishes rapport, gathers clinical data, assesses readiness for treatment; and applies criteria for diagnosis
  • Screens for psychoactive substance toxicity, intoxication, withdrawal symptoms, co-occurring mental health conditions, and risk of harm to self or others.
  • Interprets screening results to determine treatment priorities
  • Demonstrates knowledge of age-appropriate developmental milestones.
  • Formulates measurable, individualized treatment goals and objectives.
  • Applies evidence-based or accepted case management practices.
  • Demonstrates communication and advocacy skills.
  • Understands and complies with Fair Housing rules and regulations.
