About The Position

Are you passionate about helping individuals navigate mental health and recovery journeys? Henry Ford Behavioral Health Hospital is seeking a compassionate, collaborative, and dedicated Therapist/Discharge Planner to join our growing team. In this role, you'll play a vital part in supporting patients and families through treatment, discharge planning, and ongoing care coordination. Working alongside an interdisciplinary team, you'll help create individualized treatment plans that promote recovery, resilience, and long-term success. Henry Ford Behavioral Health Hospital is a joint venture between Henry Ford Health and Acadia Healthcare, bringing together nationally recognized expertise to expand access to high-quality behavioral healthcare across Southeast Michigan. Located in West Bloomfield, our state-of-the-art behavioral health hospital is designed to support both patient recovery and staff well-being. Requirements

  • Bachelor’s degree in social work, Counseling, Psychology, Human Services, or a related field required.
  • Eligible for state licensure as one of the following: Licensed Social Worker (LSW), Licensed Associate Counselor (LAC), Certified Addictions Counselor (CAC), Registered Addiction Counselor (RAC).
  • CPR certification required (training provided upon hire).
  • De-escalation/restraint certification required (training provided upon hire).
  • Strong communication, assessment, and care coordination skills.
  • Bachelor's degree in Social Work from an accredited college or university, including new graduate required.
  • CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).
  • First aid may be required based on state or facility.

Nice To Haves

  • Previous behavioral health, social services, case management, or discharge planning experience preferred.
  • One or more years’ experience working with patient population preferred.

Responsibilities

  • Conduct social history assessment of the patient through observation and patient/family interview, obtaining information on the patient’s needs, skills, interests and limitations for the development and implementation of the treatment plan.
  • Coordinate and plan programs and activities to meet patient’s social and emotional needs.
  • Promote recovery, symptom reduction and the achievement of the highest level of social functioning.
  • Provides crisis management and family sessions to assist the family in understanding the implications and complexities of a life situation and its impact on lifestyle.
  • Assist in the development of effective treatment plans that apply therapeutic principles appropriately while ensuring safety at all times for the patient.
  • Participate in multidisciplinary treatment team meeting to discuss patient assessment, treatment plan and ongoing discharge plan.
  • Communicate or aid family members in understanding and supporting the patient.
  • Manage a caseload and assume responsibility for patient chart documentation.
  • Facilitate education, process groups, support groups and referrals as requested.
  • Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
  • Perform other functions and tasks as assigned.
