This role involves providing rapid crisis intervention and assessment to individuals, utilizing face-to-face or telephone response. The therapist will work with the Access to Care team and other AltaPointe programs to ensure optimal dispositions and outcomes. Services will be provided in the community and consumer homes. The position requires effective determination of response levels in crisis situations, appropriate follow-up and referrals, and education for families on de-escalation techniques and safety plans. The therapist will also follow up with consumers in outpatient programs who may be decompensating or at high-risk, and travel to homes to ensure medication compliance as needed. Collaboration with treatment team members, law enforcement, emergency medical services, court personnel, and outside sources is essential. The role includes advocating for consumers to secure community resources and evaluating for discharge planning. Adherence to professional ethics and application of child/adolescent specific competencies are required. The therapist will also seek clinical supervision, accept feedback, and enhance clinical skills. Documentation must be timely and utilize the Avatar electronic health record, detailing crisis resolutions and linkages to follow-up sources.
