At CSR, our Behavioral Health Department (BHD) offers a comprehensive array of high quality, culturally-sensitive mental health services that provide children and families with the services they need for success at home, in school, and in the community. This role collaborates with the treatment team to implement goals and objectives in each youth’s treatment plan. The In-Home Therapist advocates for resources that support emotional and mental health needs, coordinates services with agencies such as DMH, DCF, DYS, DPH, and MCI, participates in Care Planning Team meetings when youth are enrolled in Intensive Care Coordination (ICC), and completes required documentation and data entry in a timely manner. This is a full-time role (40 hours/week) with an hourly rate of $26.45.

Requirements

  • Associate’s and/or Bachelor’s degree in a human services field with 1–2 years of experience working with children, adolescents, and transition-age youth.
  • Experience developing and facilitating training programs for families and/or staff.
  • Valid driver’s license and reliable car are required.
  • Ability to follow written and verbal directions effectively; strong communication and documentation skills.
  • Commitment to equity, cultural responsiveness, and family-centered care.

Responsibilities

  • Implement treatment plan goals and objectives in collaboration with caregivers, youth, and the treatment team.
  • Advocate for and coordinate resources that support youth and family behavioral health needs.
  • Collaborate with community and state agencies (e.g., DMH, DCF, DYS, DPH, MCI) to facilitate service access and care coordination.
  • Participate in Care Planning Team meetings when youth are enrolled in ICC; align services with wraparound principles.
  • Maintain timely, accurate documentation and complete all required data entry per payer and agency standards.
  • Provide culturally responsive, trauma-informed services that respect each family’s strengths and needs.
  • Participate in supervision, team meetings, and quality/process improvement activities.
