About The Position

Serves as the primary contact supporting individuals in the coaching program with Developmental Disabilities and Mental Health challenges for the START Program (Systematic, Therapeutic, Assessment, Resources, and Treatment) to provide community based intervention and prevention services to active participants within START. The program relies on integrated care and treatment with collaboration among all team members. The services in the START Program include collaborative contacts, after hour contacts, emergency team meetings, therapeutic coaching and therapeutic respite. We are Looking for a Therapeutic In-Home Coach to Join our Team for our START Program!!!!!

Requirements

• Bachelor’s degree in psychology, Social Work or other Human Service related field required
• 2 years’ experience with population served preferred
• Populations Served Include: Intellectual Developmental Disability, Mental Health (Child and Adult)
• Must be self-motivated, efficient with time
• Complete criminal background check
• Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
• Drug testing
• Education verification and other credentialing based on position requirements.
• Proof of employment history or references (if required)
• Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.

Responsibilities

• Coaches work within the home or other community settings to create and implement person specific and crisis therapeutic interventions and communicate these interventions with primary caregivers.
• Coaches contribute to the overall crisis stabilization and prevention of the individual and family by providing hands-on assistance and education to families.
• Coach participates as part of comprehensive START services that collaborate with individual's primary provider and clinical home, community crisis services, family and others.
• Offer consultation and hands-on education with entire family, support system to create a safe, nurturing, and meaningful environment for families and individuals with development disabilities.
• Assists individuals with implementation of daily activities, behavioral health needs, and health, wellness, and leisure activities.
• Monitors individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation support in accordance with crisis guidelines and person's plan.
• Maintains timely documentation of session and visits notes and other data.
• Coaches work with families and individuals to work on identified needs/skills to assist with strategies to improve coping mechanisms
• Coaches help to develop strategies and tools to assist others with improving overall well-being.
