About The Position

The Child and Family Support Worker is a full-time staff position, directly responsible to the Coordinator of Foster Care for fulfilling all case planning functions within the Therapeutic Foster Care Program. This role involves ensuring adherence to regulations, providing skill-building services, completing documentation, acting as a case planner, overseeing therapeutic foster homes, convening case review conferences, providing outreach and support, participating in treatment teams, maintaining statistical information, supervising visitations, providing transportation for children, and attending trainings. Specific duties include Medicaid Treatment Planning and Discharge Planning, facilitating health information exchange, coordinating Medicaid services, and accompanying children to medical appointments as a Medical Escort. The role requires participation in training commensurate with the needs of the children served.

Requirements

  • Must be 21 years of age
  • 2 years’ experience in case assessment, social work and/or service coordination
  • Travel within Steuben/Livingston service area, with reimbursement for mileage.
  • Possession of a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier.
  • Continuous use of a reliable, registered and insured vehicle.
  • Ability to prioritize assignments, plan, and complete work projects with minimal direction
  • Exceptional time management skills and ability to effectively and meet deadlines
  • Excellent verbal/written skills
  • Ability to work in the following software programs: Windows, Microsoft Office, and multiple online software platforms as required by funders and partner organizations
  • Ability to maintain confidentiality and handle confidential information with discretion
  • Ability to demonstrate behaviors and attitudes which support organizational mission, philosophy, and policies
  • Ability to work in a cooperative and helpful manner with all individuals
  • Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques

Nice To Haves

  • Bachelor’s degree in Social Worker or related field preferred
  • Prior experience in Child Welfare programs preferred
  • Relevant years of experience and education will be considered

Responsibilities

  • Ensures the rules and regulations of the NYS Office of Children and Family Services, and the Agency as they are applicable to direct services and ASFA guidelines for children and families, are adequately and appropriately adhered to.
  • Provide skill building services
  • Completes all FASPs and other supporting documents within the accepted time frame outlined by Standards.
  • Acts as a case planner, at County Request, relating to community agencies, coordinating services as needed to facilitate child and family reunification.
  • Oversees the services of the therapeutic foster home and ensures compliance with each family’s treatment plan through weekly contacts with foster parents and individual meetings with each child.
  • Convenes all case review conferences involving youth, parents, therapeutic foster parents and other service providers as needed.
  • Provides regular outreach family support and education sessions and refers family members for appropriate resources.
  • Participates on the interdisciplinary treatment team (CART) by attending weekly meetings and implementing treatment plans for children and families receiving services.
  • Maintains program statistical information as required by the Agency and all other funding sources.
  • Supervises and/or coaches visitation between family members as necessary to assure the safety and best interest of all members.
  • Provides transportation for children as necessary to fulfill the mandate of facilitating visitation between family members.
  • Attends and participates in in-service and foster parent trainings to develop and enhance social work skills.
  • Provides after-hours on-call coverage for the Therapeutic Foster Care Program (in program rotation).
  • Facilitates the exchange of health information and documentation of care received by the child in the community including attending any community-based care.
  • Coordinates Medicaid services that the child receives in the community and transition the child to home- or community-based living with outpatient treatment (e.g., individual and family therapy) as part of discharge planning.
  • If the child attends school in the community the staff coordinates care at school and delivered through the provider.
  • As needed, the Medical Escort accompanies the child to medical appointments and ensuring the child’s safety while under the Medical Escort’s supervision.
  • Participates in training that is commensurate with the intensity, scope, and duration of the needs of the children served.
