About The Position

GA Family Services is seeking a compassionate and dedicated Therapeutic Foster Care (TFC) Caseworker to provide case management, clinical support, and treatment coordination for youth placed in therapeutic foster homes and the families who care for them. This position plays a vital role in supporting children with emotional, behavioral, and mental health needs by coordinating services, facilitating treatment planning, strengthening foster family supports, and advocating for positive outcomes. The TFC Case Work collaborates with youth, foster families, biological families, community providers, schools, and referral sources to ensure comprehensive, trauma-informed care while maintaining compliance with agency, state, and regulatory requirements.

Requirements

  • Bachelor's degree required
  • Valid driver’s license, reliable transportation, and vehicle insurance as required by New York State law

Nice To Haves

  • Master's degree in Social Work, Mental Health Counseling, Psychology, Human Services, or a related field preferred.
  • Experience working with children, adolescents, and families preferred.
  • Knowledge of trauma-informed and strength-based practices preferred

Responsibilities

  • Coordinate the intake, screening, assessment, and placement of youth into therapeutic foster homes.
  • Develop, implement, and monitor individualized treatment and service plans in collaboration with youth, families, and treatment teams.
  • Maintain accurate and timely case documentation, including FASPs, court reports, progress notes, treatment plans, and other required records.
  • Conduct regular home, school, and community visits with youth and foster families to assess progress and provide ongoing support.
  • Provide crisis intervention and stabilization services as needed.
  • Serve as an advocate and liaison between youth, foster families, schools, healthcare providers, community agencies, and other service systems.
  • Facilitate and participate in multidisciplinary team meetings, treatment reviews, and permanency planning efforts.
  • Recruit, train, support, and provide ongoing guidance to therapeutic foster parents.
  • Assist with program outreach, marketing, and relationship-building with referral sources and community partners.
  • Ensure compliance with agency policies, OCFS regulations, and applicable state and federal standards.
  • Participate in the agency's 24-hour on-call rotation to provide crisis response and support.
  • Attend and successfully complete all required trainings, certifications, and professional development activities.
  • Maintain confidentiality and uphold ethical and professional standards in all aspects of service delivery.
  • Demonstrate flexibility and willingness to adapt to evolving program needs and responsibilities.
  • Maintain professional, respectful, and collaborative relationships with clients, families, community partners, and colleagues.
  • Demonstrate ethical and professional conduct while upholding confidentiality standards.
  • Communicate effectively, both verbally and in writing.
  • Participate in regular supervision, staff meetings, and performance evaluations.
  • Support the agency's mission, values, and organizational goals.
  • Uphold child and family rights in accordance with OCFS regulations and agency procedures.
  • Follow all agency safety policies and procedures.
