About The Position

The Territory Patient Admission Coordinator is responsible for the timely acceptance or decline of the referral based of the predetermined criteria set by the Agency. This position’s assigned territory, may encompass one or more states in AccentCare’s operation and may change with the needs of the Company. The Territory Patient Admission Coordinator establishes and maintains communication with the clients, family members and referral sources. This role is responsible for partnering with agency staff, referral sources, and sales to achieve optimal patient satisfaction & outcomes. The Territory Patient Admission Coordinator facilitates obtaining all information and documentation necessary to complete a referral through follow up with the Agency, the Referral Source and the Client.

Requirements

  • Previous Backoffice experience required
  • Current LVN License required for consideration
  • Licensed LVN or RN in practicing state.
  • Minimum of one year experience in home care, hospice, or personal care services, preferred
  • Prior use of Electronic Medical Records (EMR), preferred
  • Experience in dealing with a variety of payors in healthcare, e.g., Medicare, Medicaid, and commercial payers, preferred.

Nice To Haves

  • 3-7 years of experience in facility/physician relationships with a deep understanding of facility discharge processes, preferred.

Responsibilities

  • Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
  • Obtains & Documents Verbal Orders for patient evaluation, as required in state in which the Territory Patient Admission Coordinator is licensed.
  • Documents all referral related Information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable utilizes the agency’s software solution to facilitate the referral process by completing required workflow task within the Company’s established time frames.
  • Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal.
  • Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients.
  • Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.
