UPMC Health Plan is seeking an experienced nurse with a background in case management for a full-time Telephonic Care Manager position supporting the Special Needs Plan team. This role is predominantly work-from-home, operating during standard EST daylight hours, Monday through Friday. The position involves care coordination and health education for Health Plan members through telephonic collaboration with members, caregivers, and providers. Responsibilities include identifying medical, behavioral, and social needs and barriers to care, developing comprehensive care plans to close preventive care gaps and support chronic illness self-management, and collaborating with various stakeholders to provide additional support. Members are contacted via telephone or other electronic communication methods. The title and salary will be determined based on education and nursing experience, with potential for a Sr. Professional Care Manager role within the Insurance Services Division.
