About The Position

The Care Coach 1 assesses and evaluates member's needs and requirements to achieve and/or maintain an optimal wellness state by guiding members/families toward and facilitating interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 employs a variety of strategies, approaches, and techniques to manage a member's health issues telephonically. This role identifies and resolves barriers that hinder effective care and ensures the patient is progressing towards desired outcomes by continuously monitoring patient care through the use of assessments, data, conversations with the member, and active care planning. The Care Coach 1 understands their work area's professional concepts, standards, regulations, strategies, and operating standards. They make decisions regarding their own work approach and priorities, and follow direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation, and follows established guidelines/procedures.

Requirements

  • Bachelor’s degree in a health or social service-related field or LPN licensure in Illinois.
  • 2 or more years of relevant/case management experience.
  • Experience utilizing Microsoft Word, Excel and Outlook.
  • Experience with electronic information applications/software programs.
  • Exceptional verbal/written communication and interpersonal skills.
  • Minimum download speed of 25 Mbps and an upload speed of 10 Mbps for internet service.
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Nice To Haves

  • Bilingual English/Spanish/Polish/Arabic
  • Certified Case Manager (CCM)
  • Master's degree in a health or social service-related field.
  • Prior experience with Medicare & Medicaid recipients.
  • Previous experience with electronic case note documentation and experience with documenting in multiple computer applications/systems.
  • Experience with health promotion, coaching and wellness.
  • Knowledge of community health and social service agencies and additional community resources.
  • Experience with case management, discharge planning and patient education for adult acute care.
  • Preferred residing in IL.
