About The Position

The Telephone Triage Nurse provides patient support through telephonic or electronic communication and assists in coordination of care through multiple service lines. This position prioritizes information as it is presented through a variety of sources.

Requirements

Knowledge of standard nursing procedures and chronic condition education.
Strong customer service and the ability to maintain a positive attitude under pressure.
Ability to communicate effectively both oral/written with a broad base of stakeholders.
Attention to detail and a high level of accuracy and confidentiality.
Ability to think critically, solve problems, and work independently with minimal supervision.
Moderate level of desktop computer skills to include the O365 productivity software suite.
In-depth knowledge and efficiency with EHR systems.
A High School Diploma or GED is required.
Graduation from an accredited RN or LPN program is required.
Active RN or LPN license in the State of Tennessee is required.
BLS certification is required.

Nice To Haves

Outpatient experience is preferred.

Responsibilities

Triage patients with both acute and chronic needs via telephone or other electronic communication and determine the next steps of action.
Documents encounters in the EHR in a timely manner to allow for follow up and gap closure.
Assist in coordinating care through multiple service lines and various departments.
Assist with a variety of paperwork involving care coordination.
