This full-time position involves providing patient/family-centered care as a partner and advisor, assisting patients and families in optimally managing their healthcare. The role requires applying critical reasoning and clinical judgment to expedite appropriate care and treatment, assessing patients through telecommunication technologies such as telephone, fax, email, internet, or patient portal. The nurse will also ensure proper scheduling and follow-up on test results for providers, use active listening skills, approved protocols, and evidence-based practice to explain test results and medications, and adhere to the American Nurses Association Code of Ethics for Nurses. Participation in professional development, maintaining competencies, identifying learning needs, and pursuing lifelong learning are essential. The role focuses on patient safety and quality of nursing care through appropriate interventions, including health education, patient advocacy, coordinating services, assisting patients in navigating the healthcare system, and evaluating outcomes. Maintaining knowledge of care teams for coordination, self-management, population management, and communication skills is also key. The nurse will adapt communication styles to meet diverse patient needs and demonstrate exceptional assessment, critical thinking, and customer service skills. Other related duties incidental to the work described may be performed.
