About The Position

Belle is redefining primary care for Medicare members by providing in-home preventive foot care through Community Health Workers, who are licensed nail technicians. This approach builds trusted relationships and identifies health issues earlier, connecting members to Chronic Care Management and extending the clinical team's reach via AI-powered telehealth and care coordination. Requirements

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license in Georgia
• Credentialed with major plans: UHC, Aetna, Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
• Collaborative, mission-driven mindset with excellent communication skills

• Multi-state licensure a plus

Responsibilities

• Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
• Create and manage personalized care plans for ongoing conditions
• Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
• Ensure thorough documentation in our EHR and compliance with quality measures
• Engage patients in preventive care to reduce avoidable complications
