About The Position

The Regional Team Facilitator supports clinical and operational functions across the region, providing collaborative assistance to both field clinicians and office-based teams. This role helps ensure the accuracy, integrity, and compliance of electronic medical records (EMRs) and clinical documentation. The Team Facilitator promotes efficient workflows, supports regulatory standards, and enhances continuity of patient care through coordinated oversight and operational support.

Requirements

  • High School diploma or GED
  • Successful completion of a CT state approved LPN certificate program
  • 1 year experience in healthcare
  • Current and valid LPN license
  • Valid CT driver’s license & a good driving history

Responsibilities

  • Oversee and coordinate the Resumption of Care (ROC) process, including tracking patients on transfer and ensuring timely communication to all departments regarding transitions back home.
  • Monitor and manage coding queries and ensure timely, accurate responses to meet coding team requirements.
  • Oversee and manage wound care orders by monitoring incoming orders, conducting weekly reviews and comparisons, ensuring care teams are informed of any updates, and maintaining accurate documentation in HCHB, including order entry and transcription as needed.
  • Ensure accurate clinical review and routing of physician orders by monitoring Forcura for misassigned returns, reviewing patient charts, and reassigning orders to the appropriate physician as needed.
  • Coordinate SBAR communication and follow-up by reviewing clinician submissions, ensuring accurate documentation in patient charts, and facilitating necessary actions such as medication reconciliation and Memo of Understanding (MOU) completion, while supporting overall office operations and serving as a resource to field clinicians through timely administrative and clinical support.
  • Monitors inventory levels and coordinates the ordering of necessary supplies, including wound care materials, car stock, and general office supplies, to ensure uninterrupted operations.
