About The Position

The Teacher, Special Education Transition provides students with support and specially designed instruction to assist students in developing the competencies with their post-secondary goals. These post-secondary goals could include goals related to careers, as well as goals related to independent living, and are aligned with the student’s individualized education program (IEP). This position reports to a Senior Manager, Special Education.

Responsibilities

  • Assists with the development and progress toward mastery of transition goals and objectives in the implementation of the IEP.
  • Identifies/develops job opportunities, assists students with completion of applications, trains students on appropriate work-related behaviors and social skills, participates in job interviews, and provides job coaching according to the individual needs of the student.
  • Maintains a regular schedule of direct involvement with the student in the implementation of the student’s IEP in partnership with community partners.
  • Provides a continuum of supports and services to community partners for the students including: internship collaborations, ongoing communication, and on-the-job supports and job coaching.
  • Assists with evaluation of student vocational skills and behavior to assist in determining work readiness, abilities, interests, and preferences.
  • Collaborates with parents, the student, and community partners in the development, implementation, alignment, and mastery of transition goals and objectives.
  • Networks with local business and community leaders to gain knowledge of the job market in the community and help secure job sites for student placement.
  • Leads and participates in IEP meetings for students assigned to designated caseload.
  • Prepares and maintains IEP documents in a timely manner, including progress updates.
  • Plans lessons that reflect accommodations and modifications for individual differences.
  • Implements positive behavioral interventions and supports with discipline management procedures.
  • Plans and supervises purposeful assignments for teacher assistants and/or employment representatives.
  • Other duties as assigned.
