About The Position

Trillium Health Resources is seeking a Transition to Community Living (TCL) Housing Supervisor to oversee a team of Housing Specialists. This role is crucial in supporting individuals with serious mental health needs, substance use disorders, traumatic brain injuries, and intellectual/developmental disabilities in acquiring and maintaining supportive housing. The team provides care coordination activities including treatment planning, assessment, monitoring, education, and referral/linking services for members transitioning to supportive housing, at risk of eviction, or currently facing eviction. The TCL Housing Supervisor ensures members receive comprehensive, whole-person care aimed at achieving positive outcomes.

Requirements

  • Bachelor’s degree in a Human Services field and five (5) years of experience working in care management, case management or care coordination with the MH/SU/BH population, including experience working with individuals and the housing system serving this population to obtain and maintain permanent supportive housing; OR Master’s degree in a Human Services field and three (3) years of experience working in care management, case management or care coordination with the MH/SU/BH population, including experience working with individuals and the housing system serving this population to obtain and maintain permanent supportive housing.
  • Must have a valid driver’s license.
  • Must reside within Trillium’s catchment area of North Carolina, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson.
  • Must be able to travel within catchment as required.

Responsibilities

  • Provide direct supervision and oversight for assigned team.
  • Ensure the appropriate completion of Housing Assistance Payment (HAP) contracts, including monthly review of invoices.
  • Monitor and reconcile housing data in the North Carolina Housing and Finance Agency (NCHFA) platform on an ongoing basis.
  • Ensure outstanding tenancy issues are addressed in a timely manner.
  • Oversee the housing inspection referral process with a third-party inspection vendor.
  • Lead meetings to provide updates on outstanding inspection referrals.
  • Provide technical assistance to Housing Specialists and landlords regarding specific member needs.
