The Adult Case Manager is a bachelors prepared professional Certified in Behavioral Health Case Management (BHCM). The Case Manager works as a part of the collaborative care team of healthcare professionals led by a Department of Health primary care provider to support adults with mental health and substance use disorders served in the medical home and assessed and determined to be in need of and will benefit from a low intensity level of care as determined by the LOCUS. The Case Manger plays an integral role of support, linkage, coaching, and guidance for adults with a mental health or co-occurring mental health & substance abuse diagnosis and who have been assessed and determined to be in need of and will benefit from a low intensity level of care as determined by the LOCUS level 1 or 2 as part of Health Care for the Homeless (HCH), and PCHP. Case management services focus providing assistance with arranging financial support, supportive housing, systems management, and transportation, utilizing natural supports, community supports, and input and resources from other community partners or professional agencies. Case management services occur in the community at a place convenient and comfortable for the adult. This is a field-based position and reports to the Supervisor. Directions for Living is an essential services provider. All Directions for Living Team Members must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation.
