About The Position

Easterseals PORT Health is seeking a compassionate and committed Tailored Care Manager to join its multidisciplinary Tailored Care Management (TCM) team in Plymouth, North Carolina. The role supports individuals affected by mental health and substance use challenges through whole-person, integrated care. The manager evaluates client needs, develops personalized care plans, and coordinates services addressing physical and mental health, social determinants of health, and other life challenges. The position involves hands-on community-based work and travel throughout Plymouth and surrounding counties to help close gaps in care. It is full-time, primarily Monday through Friday, 8 AM to 5 PM, with rotating on-call responsibilities and services delivered in varied community settings.

Requirements

• Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area.
• Eligible for Qualified Mental Health Professional (QMHP) credentialing.
• At least 2 years of experience with mental health and/or substance use populations.
• Proficiency in Microsoft Office and Electronic Health Records (EHR).
• Valid driver’s license, current auto insurance, and a clean driving record.
• Strong communication skills and ability to work independently.
• Flexibility to provide services in various community settings and participate in rotating after-hours support.
