About The Position

Easterseals PORT Health's Tailored Care Management (TCM) team is committed to delivering whole-person, integrated care that truly makes a difference. As a Tailored Care Manager, you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands-on role with the opportunity to travel throughout Wilmington, NC and surrounding counties, helping close gaps in care and improving lives in the community every day.

Requirements

Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area.
Eligible for Qualified Mental Health Professional (QMHP) credentialing.
Minimum 2 years of experience with mental health and/or substance use populations.
Proficiency in Microsoft Office and Electronic Health Records (EHR).
Valid driver’s license, current auto insurance, and a clean driving record.
Strong communication skills.
Ability to work independently.

Nice To Haves

Master’s level degree preferred.
