- Experience in healthcare, customer service, call centers, or care coordination is a plus
Responsibilities
- Make outbound and receive inbound calls with members and healthcare partners.
- Build positive relationships through compassionate, professional communication.
- Assist Tailored Care Managers by completing outreach, scheduling appointments, coordinating services, and documenting member interactions.
- Help connect members with healthcare providers, community resources, and essential support services.
- Maintain accurate documentation while meeting quality and productivity goals.
