About The Position

This is a full-time, direct care position. The primary responsibilities of the Extender are to perform general outreach, engagement, and follow-up with members, coordinate services/appointments, engage clients in health promotion activities, share information with the Care Manager on the member's circumstances, make and track referrals, participate in case conferences, and support the Care Manager in assessing and addressing unmet health-related resource needs.

Requirements

• Must be at least 18 years of age
• High school diploma or equivalent
• Two (2) years paid experience performing the following types of functions
• At least one year of paid experience working directly with behavioral/mental health clients

Nice To Haves

• Spanish language proficiency preferred
