SWITCH Case Manager - Memphis Allies
About The Position
Memphis Allies, launched by Youth Villages, is a collaborative initiative designed to reduce gun violence and build safer neighborhoods in Memphis. The program focuses on providing opportunities for adults, youth, and their families while addressing the root causes of violence. The Support with Intent To Create Hope (SWITCH) model combines street outreach, life coaching, clinical services, and case management to support individuals at high risk of involvement in gun violence. The goal is to build stability and safety, change lives, reduce gun violence, and strengthen communities.
Requirements
- High School diploma or equivalent (required)
- Must have lived in Memphis, TN for a minimum of two years (required)
- Credibility and cultural competency (required)
- Applicants must possess a current, valid driver’s license, an automobile for work purposes, and proof of auto insurance.
- Basic computer knowledge
- Ability to maintain a flexible schedule
Nice To Haves
- Bachelor's degree (preferred)
- Experience with case management (preferred)
- Experience working with youth and families in need of crisis intervention (preferred)
- Strong organizational skills and attention to detail
- Excellent written, verbal, and oral skills
- Ability to manage multiple priorities simultaneously
Responsibilities
- Carries a caseload of approximately 24 youth and young adults.
- Conducts weekly in-person sessions as needed.
- Provides therapeutic services in the home and other community-based locations.
- Connects youth, young adults, families, and caregivers with resources.
- Collaborates with other partners and services involved with the youth, young adult, family, and caregivers.
- Provides accurate and detailed information to the Supervisor in a timely manner in the event of a crisis.
- Adheres to guidelines set as a mandated reporter under the guidance of the Clinical Supervisor, Regional Supervisor, and Licensed Program Expert.
- Adheres to Youth Villages' confidentiality, ethical, and legal guidelines.
- Develops and implements a Professional Development Plan to increase skills for professional growth.
- Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development and quarterly booster training sessions.
- Provides on-call crisis support to youth and adults (schedules vary by location but require 24/7 availability).
- Completes accurate and timely documentation in an electronic medical record system (EMR).
- Performs other duties as assigned.
