About The Position
Ascension is seeking a Surgical Nurse Navigator in Baltimore, MD. This full-time, day shift position within the Surgery General department at St. Agnes Healthcare involves navigating patients through their healthcare journeys, serving as a liaison between providers, insurers, and community organizations, coordinating treatment efforts and quality benchmarks, and managing patient and staff education. The role also includes coordinating follow-up care and social services for underinsured and uninsured individuals.
Requirements
- Registered Nurse credentialed from the Maryland Board of Nursing obtained prior to hire date or job transfer date required.
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
Nice To Haves
- BLS Provider credentialed from the American Heart Association (AHA) preferred.
Responsibilities
- Navigate patients through complex healthcare journeys to ensure continuity of care.
- Serve as the central point of contact for providers, insurers, and community organizations.
