Surgery Clinical Pharmacist - Day Shift
About The Position
Vanderbilt University Medical Center (VUMC) is seeking a Surgery Clinical Pharmacist to provide leadership in a specialized healthcare team and to patients, ensuring optimal and cost-effective drug therapy. This role involves developing pharmacotherapeutic goals, recommending regimens, and creating monitoring plans tailored to individual patient needs. The clinical pharmacist will lead efforts in preventing and reporting medication errors and adverse drug reactions, contributing to continuous improvement in medication safety practices within the institution. This position will primarily cover shifts for other surgery pharmacists during their time off, offering exposure to various patient care teams. Order verification will also be a responsibility. Clinical services include daily patient review, interdisciplinary team rounding, serving as a drug information resource, code response, completing pharmacy consults, supporting transitions of care, patient education, antimicrobial stewardship, and monitoring high-risk medications. The shift is Day Shift, from 7:00 am to 3:30 pm, with every 6th weekend and holiday rotation.
Requirements
- LIC-Pharmacist License - Tennessee
- Relevant Work Experience
- 1 year experience
- Bachelor's degree

Responsibilities
- Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
- Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
- Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
- Maintains the quality, safety, and consistency of clinical Pharmacy services in assigned areas.
- Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
- Daily patient review.
- Interdisciplinary team rounding based on need.
- Serving as a drug information resource for providers, nurses and patients.
- Code response.
- Completion of pharmacy consults.
- Support at transitions of care.
- Patient education.
