About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Surgery Clinical Pharmacist to join their Pharmacy Clinical Team Lead 3. This role provides leadership in a specialized healthcare team to ensure optimal, cost-effective drug therapy. The pharmacist will develop pharmacotherapeutic goals, recommend regimens, and create monitoring plans for individualized patient needs. A key responsibility includes leading efforts in preventing and reporting medication errors and adverse drug reactions, contributing to continuous process and practice improvement to enhance medication therapy safety. This position will primarily cover surgery pharmacists' time off, offering exposure to various patient care teams, and will also involve order verification. Clinical services include daily patient review, interdisciplinary team rounding, drug information resource provision, code response, pharmacy consults, transitions of care support, patient education, antimicrobial stewardship, and monitoring of high-risk medications. The shift is Day Shift, from 7:00 am to 3:30 pm, with every 6th weekend and holiday rotation.

Requirements

  • LIC-Pharmacist License - Tennessee
  • Relevant Work Experience
  • 1 year experience
  • Bachelor's Degree (Required)

Responsibilities

  • Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications.
  • Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy.
  • Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
  • Maintains the quality, safety, and consistency of clinical Pharmacy services in assigned areas.
  • Supervises the activities of assigned support personnel and verifying the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
  • Daily patient review.
  • Interdisciplinary team rounding based on need.
  • Serving as a drug information resource for providers, nurses and patients.
  • Code response.
  • Completion of pharmacy consults.
  • Support at transitions of care.
  • Patient education.
  • Antimicrobial stewardship.
  • Monitoring of high-risk medication therapies.
  • Order verification responsibility.
