About The Position

Under the direction of a Coordinator or Supervisor within the Life Consultation Unit of CSDD, provides community-based direct and indirect support to individuals to create and implement a self-directed arrangement. The right candidate will have a desire to support people within their person-centered plan, a willingness to engage with the person’s family and friends and Case Manager within the utilization of Medicaid funded supports. We treat to need and support people through a variety of situations and different environments. Additional responsibilities include consultation with people and their teams, collateral activities on behalf of the person, detailed documentation, budget creation, and training. Responsible for carrying out all activities of the program/sub-unit in such a manner that fulfills CMHA-CEI’s mission, policies and procedures.

Requirements

  • Possession of a Bachelor's Degree in Social Work, Psychology, or closely related field required.
  • Possession of an appropriate State of Michigan credential (e.g., LLBSW, LBSW, SSTR) is required.
  • One year of professional work experience with persons with developmental disabilities is required.
  • Ability to communicate accurately and effectively both in writing and verbally required.
  • Possession of a valid Michigan driver’s license and access to reliable transportation for job related use is required.
  • Must have ability to operate a computer, and gain proficiency in Care Manager, the agency software for service authorizations, service entry, and billing for Contract Providers.
  • Must be physically capable of interacting with and/or transporting consumers of various height and weight.
  • Ability to tolerate exposure to body fluids and risk of consumer aggression is required.
  • May be required to be fingerprinted.
