About The Position

Thee Agency is seeking a compassionate, organized, and dedicated Supports Broker to join our growing team. The Supports Broker will work alongside individuals receiving self-directed services through the New Jersey Division of Developmental Disabilities (DDD) to identify goals, navigate resources, and build meaningful plans that promote independence, choice, and community inclusion. This position is ideal for someone who is passionate about advocacy, enjoys building relationships, and understands the importance of supporting individuals to lead self-directed lives.

Requirements

  • Strong communication, organization, and problem-solving skills.
  • Ability to build trusting relationships with individuals and families.
  • Comfortable working independently while maintaining strong communication with the agency team.
  • Proficient with email, Microsoft Office, and electronic documentation systems.
  • Valid driver's license, reliable transportation, and willingness to travel throughout New Jersey.

Nice To Haves

  • Experience working with individuals with intellectual and developmental disabilities preferred.
  • Knowledge of NJ DDD services, self-directed supports, and community resources preferred.
  • Supports Brokerage experience required
  • Must have completed Boggs - Orientation to Supports Brokerage
  • Experience with DDD Individualized Service Plans (ISPs).
  • Experience assisting individuals with housing goals and community integration.
  • Background in social services, case management, advocacy, or related fields.

Responsibilities

  • Work directly with individuals and families to identify personal goals, preferences, and desired outcomes.
  • Provide guidance and support related to self-directed services and available resources.
  • Assist with navigating community resources, including housing opportunities, transportation, employment supports, and recreational opportunities.
  • Support individuals through the housing search process, including identifying options, coordinating with housing resources, and assisting with applications when applicable.
  • Advocate for individuals while ensuring their choices and preferences remain at the center of decision-making.
  • Maintain communication with individuals, families, Support Coordinators, providers, and other involved parties.
  • Complete required documentation, progress notes, service logs, and reports accurately and timely.
  • Assist individuals with understanding and maximizing their approved self-directed budgets.
  • Attend meetings and collaborate with the individual's support team as needed.
