The Coordinator of Supportive Housing reports to the Director of Supportive Housing and functions according to the policies established by the Board of Directors. This role is an exciting opportunity in leadership for Favarh and is responsible for the overall management of the Supportive Housing Program. The Coordinator supervises all assigned Supportive Housing Managers directly. Facilitates regular file reviews of assigned programs to ensure compliance with agency and state policy and procedures. Assists with referrals, proposals, and intake for assigned Supportive Housing programs, as well as participate in interviewing and hiring of all program staff. In addition, the Coordinator may provide support as Acting Director in the absence of the Director of Supportive Housing.

Requirements

  • Bachelor's Degree and five years of applicable experience including staff supervision or, a total of ten years of applicable experience including at least seven years of increasingly responsible supervisory and management assignments encompassing program planning, staff supervision, and day to day operational management.
  • Medical Administration Certification: required within 3 months of hire to retain position.
  • PMT certification is required within 6 months of hire to retain position.
  • Must complete all residential training within licensing required timelines.
  • Must be physically able to implement restraint procedures and administer CPR without reasonable anticipation of injuring oneself or others.
  • Must have a valid State of Connecticut Driver's License and obtain a State of Connecticut Public Services Driver's License when applicable within 6 months of hire and be insurable with Agency carrier without additional expense to Agency.
  • Must be able to produce a current/valid Health Certificate

Responsibilities

  • Follows procedures in a manner that respects the human and civil rights of individuals and staff. Encourages and advocates for individual in the IHS program to the highest level of self-advocacy desired.
  • Demonstrates effective interpersonal skills which enhance communication between individuals and staff.
  • Provides supervision and support to assigned managers including regular team meetings, participation in assigned individual IP meetings, training oversight and habilitative process oversight.
  • Must demonstrate the ability to coordinate complex systems and communicate effectively verbally and in writing with federal, state, and local systems, and with Favarh staff.
  • Provides the Director of Supportive Housing information and recommendations regarding Supportive Housing program issues.
  • Ensures support, training and education to both staff and individuals to provide the safest environment possible while allowing dignity of risk.
  • Possess thorough knowledge on existing quality assurance/quality service regulations per DDS policy and ensures follow up/response to all citations. Provides routine random file and site reviews to ensure quality assurance.
  • Provides financial oversight for all revenues and expenses in the Assigned Supportive Housing programs including waiver billing and one time funding.
  • Maximizes funding by ensuring LONs are updated and that the agency is receiving maximum amount through safety net/cluster funding, behavior support, rental assistance subsidies, heating assistance and DSS funding/benefits.
  • Ensures adequate services and subsequent documentation is in place to ensure billing is accurate and complete.
  • Assists in responsibility of Department oversight in Director of Supportive Housing’s Absence as assigned.
  • Assists the Director of Supportive Housing in the formulation of policies and procedures and annual Department Goals.
  • Provides regular visits to program sites when individuals are at home to ensure consumer satisfaction.
  • The Coordinator of Supportive Housing is required to be available by cell phone 24/7 to program staff/individuals.
  • Serves on Agency committees on a voluntary basis or as assigned.
  • Maintains sound Interdepartmental Communication.
  • Actively participates and encourages department participation in Agency special events and fund-raising activities.
