About The Position

Established in 1948, The Mission of ASCEND Mental Wellness is to improve the quality of life for those affected by mental illness and to promote the awareness and importance of mental well-being in the community. We are seeking a Supportive Housing Residential Coordinator to join our leadership team. In this role, the candidate will oversee our Housing First Program being responsible for the day-to-day functioning and operation of the Supportive Housing Residential Programs including overseeing staff meetings, finances, supervision of activities and provision of services to residents. Housing First provides safe, affordable, permanent housing for 18 individuals with histories of being homeless as well as being disabled by mental illness and/or substance abuse. The priority of this housing model is to provide stable housing in an empowering, supportive environment with the goals of increasing residential stability, enhancing tenant skills and fostering greater self-determination. The program consists of 12 one bedroom and 6 studio apartments, a community area and a large fenced in yard. Along with that, our Housing First program has a four bed Interim Room for Adult females aged 18 and older.

Requirements

  • Bachelor's degree in Human Services related field preferred along with experience working in human services field, including supervisory experience.
  • Associate's Degree with at least five years' experience working in human services field, including supervisory experience.
  • Valid NYS driver's license acceptable to Agency insurance standards.

Responsibilities

  • Evaluate resident referrals, conduct admission meetings w/supervisor and assist in the development of service plans and discharges for residents as needed.
  • Ensure and advocate that residents are receiving appropriate services with the program and that referrals are made for services outside the program when indicated, networking with community providers.
  • Implement and actively participate in selected program activities and be available to residents.
  • Participate in the development of program policies and procedures.
  • Be responsible for the required statistical and reporting documentation in the program (i.e. goal plans, yearly reports, fire drills, rep payee, etc.)
  • Be responsible for rent calculations, collection and working collaboratively with the Accounting department.
  • Encourage and facilitate resident input and participation in the development and maintenance of program offerings.
  • Facilitate resident meetings.
  • Provide and oversee orientation of new staff.
  • Ensure timely compliance with all required trainings.
  • Provide regular supervision and evaluation of Assistant Coordinators, Residential Specialists, Relief Residential Specialists, and Residential Nurse.
  • Oversee the physical condition of the residence(s); make/approve small repairs as needed, inform supervisor of larger maintenance problems.
  • Be On-Call as assigned.
  • Provide supportive counseling to residents as appropriate; encourage and model effective communication, personal and interpersonal awareness and client centered approach.
  • Familiarity with resident entitlements, including SSI, HUD, sources of potential funding and eligibility requirements for services to residents.
  • Assist in program certification/re-certification processes as required.
  • Represent the Agency and the Supportive Housing program by maintaining good working relationships and open communication with other agencies, services, and individuals in the community, and facilitate coordinated service provision for residents through them.
  • Maintain a safe environment for all residents and staff.
  • Maintain effective communication with other staff members and supervisor.
  • Preserve human dignity and confidentiality of all residents and act in the best interest of those served through the Agency's programs and services.
  • Assure quality of work at all times.
  • Adhere to all laws and regulations applicable to the Agency's business.
  • Adhere to the Agency's Corporate Compliance Program and established policies and procedures.
  • Positively represent the Agency within the community at all times.
  • Have adequate computer skills, or more refined for specific job titles.
  • Actively participate in scheduled team/staff meetings as scheduled by supervisor or as indicated by the program's services.
  • Actively participate in Agency sponsored staff development and in-service educational programs as deemed necessary by supervisor, including mandatory trainings assigned per job title.
