To demonstrate support of the hospital mission and vision by providing nursing, case management/disease management services to clients and families to help in the coordination and management of individuals’ health needs through the full continuum. The Registered Nurse demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes. Performs assessment of clinical and life issues for the patients of COH. Provides professional nursing services in support of the physician, in the direct and indirect care of patients. The successful candidate: Must be able to work Monday-Friday, 8a-4:30p

Requirements

  • Current Georgia RN license from an accredited college
  • BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date
  • BLS (Obtain AHA certification within 30 days of hire)
  • Must be able to work Monday-Friday, 8a-4:30p

Responsibilities

  • Provides oncology-focused disease management, including monitoring chemotherapy treatments, side effects, and symptom management
  • Educates patients and families on diagnosis, treatment plans, and quality-of-life considerations throughout the continuum of care
  • Conducts patient assessments, toxicity screenings, and documents interventions accurately in the electronic medical record
  • Collaborates closely with physicians and interdisciplinary teams to coordinate and manage patient care
  • Triage patient concerns (in-person and via phone), implementing appropriate interventions and escalating issues as needed
  • Serves as a key communication liaison between patients, families, providers, and external care services
  • Coordinates care plans, including home services and medical equipment, to support patient needs
  • Delivers timely, patient-centered care while ensuring high levels of service, responsiveness, and patient satisfaction
