Requirements

  • Graduate of an NLN accredited school of Nursing.
  • Current RN licensure in the Commonwealth of MA and RI is required.
  • Two years recent hospital, medical/surgical or other relevant nursing experience required.
  • Evidence of current CPR certification.
  • Must possess and demonstrate highly effective interpersonal, customer service, problem solving and communication skills.
  • Requires proficient computer skills, including experience with various software, database and electronic records programs.
  • Able to effectively utilize electronic devices to communicate with other members of the hospice care team.
  • Able to effectively use electronic medical record to document patient assessments, plan of care, care and services provided.
  • Current RN licensure in the Commonwealth of MA and RI is required.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired

Nice To Haves

  • Bachelor of Science in Nursing or currently enrolled in a program leading to a B.S.N. is preferred.
  • Hospice, Palliative Care or Homecare experience preferred.
  • Hospice & Palliative Care Certification is preferred and will be required within eligibility timeframe if not certified at time of hire.

Responsibilities

  • Under the supervision of the Director-Supportive Care Services or designee, and within agency and department policies and procedures, duties include, but are not limited to: providing the consistency and predictability necessary to transition a patient's care between care settings or levels of care, coordinating the care transition for patients requiring the services of hospice, DME, Pharmacy and infusion services, assessing hospice eligibility, completing part or all of the admission process, providing daily GIP visit services, orienting patients and families to hospice services and establishing and maintaining professional relationships within the assigned facilities.
