About The Position

The Permanent Housing Care Coordinator is responsible for working with TAY families at the Richard Lehn Intergenerational Campus, located in Santa Ana. This position also includes providing Care Coordination with young adults exiting any of Orangewood’s housing programs and supporting their permanency in the community through the Permanent Supportive Housing Care Coordination model as well as through the Housing Aftercare Program. Requirements

  • Requirements for Coordinator II: Requires a High School Diploma with a minimum of 4 years’ experience or a bachelor’s degree with 2 years of experience with similar case management services.
  • Requirements for Coordinator III: High School Diploma with 6 years of experience or a bachelor’s degree with 3 years of experience or a master’s degree in social work or related field with internship plus six or more months experience in case management.
  • Minimum 3yr experience in the human services field, higher education, working with homeless transitional age youth, current and former foster youth, counseling, or relevant experience
  • Demonstrate a basic understanding and be comfortable working with youth who present with, substance use, mental and behavioral health, and/or are homeless.
  • Possess knowledge and understanding of higher education, financial aid, scholarship and grants.
  • Be comfortable with and understand adolescent & child abuse issues.
  • Strong written and oral communication skills.
  • Must be able to engage with partner organizations and be comfortable speaking on the phone, presenting oneself in professional and community settings.
  • Demonstrate strong, independent project management skills.
  • Ability to multi-task, be detailed oriented, independently make significant decisions, and be a self-starter.
  • Proficient in Microsoft Office applications; Outlook, Word, Excel, and Power Point.
  • Bilingual in English and a county threshold language preferred (Spanish, Vietnamese, Korean, Arabic, Farsi, Chinese (Simplified), and Russian).
  • Possess a valid California driver’s license and proof of automobile insurance. Position requires DMV clearance.

Responsibilities

  • Complete initial and on-going assessment of youth for eligibility and needs to set goals and action plans.
  • Work closely with property management to maintain positive landlord-tenant relationships and prevent evictions.
  • Provide ongoing care coordination, including home visits and check-ins that promote safe, stable housing and family functioning.
  • Facilitate and connect families to parenting support groups, life skills workshops, budgeting, including financial literacy.
  • Support youth and families in budgeting, lease compliance, and building/reinforcing independent living skills.
  • Participate in case conferencing and multidisciplinary team meetings to align services.
  • Serve assigned caseload of up to 20 youth both on site and in Aftercare.
  • Provide crisis intervention and link youth and families to appropriate crisis services.
  • Connect youth with Orangewood and community services such as childcare, medical & mental health treatment, public assistance, and other applicable services.
  • Serve as a liaison between the youth, family, community partners, and program staff, ensuring wraparound support.
  • Maintain contact with stakeholders such as social workers, county workers, caregivers, school personnel, etc.
  • Collect and track data as needed for reporting and meeting project goals and objectives.
  • Maintain accurate client files, good recordkeeping, update as needed and adhere to confidentially policies.
  • Maintain current knowledge of CalOptima Health CalAIM rules and regulations.
  • Ensure confidentiality of client files per HIPAA and all applicable guidelines.
  • Ability, desire, and flexibility to work in a high crisis, fast paced environment.
  • Conduct outreach to organizations (colleges, universities, and businesses) and personnel interested and able to assist youth
  • Provide outstanding customer service
  • Additional duties as assigned by direct supervisor.
