About The Position

The Supported Housing Program Manager is responsible for the strategic and operational aspects of care delivery, including comprehensive care planning, interdisciplinary coordination, resource management, ongoing evaluation of resident needs and services, and the planning, development, implementation, and evaluation of nursing care services. The Supported Housing Program Manager assesses, plans, implements, and evaluates resident care; works collaboratively with the interdisciplinary team to ensure the delivery of safe, consistent, and person-centered care; and provides leadership, direction, and guidance to LPNs and HCAs in the provision of resident care. The Supported Housing Program Manager provides support, advocacy, and choice to residents and their families to ensure the highest quality of person-centered care achievable within the mission, vision, and values of The Salvation Army. The Supported Housing Program Manager supports their Director in providing continuing care and assists in maintaining a safe and secure environment for residents, visitors, and staff members, while also supporting quality improvement and the effective coordination and ongoing evaluation of care practices and resources within the program.

Requirements

  • Possess a Regulated Health Care Professional Qualification (LPN, OT, RN or PPN)
  • Equivalent combination of education and experience may be considered.
  • Current certification in BLS required.
  • Criminal Record Check with Vulnerable Sector Check required.
  • Minimum 3 – 5 years of relevant work experience in a Supportive Living site.
  • Knowledge of the Continuing Care Health Services Standards (for supportive living).
  • Experience and passion about working with marginalized persons who may have addictions and mental health issues.
  • Working knowledge of Trauma informed practice, cultural competency, recovery, addictions, mental health, and self-determinants of health.
  • Good organizational and time management skills with the ability to multi-task.
  • Must possess excellent communication (written and verbal) and interpersonal skills. dealing with residents and staff members.
  • Possess critical thinking, problem solving, and decision making.
  • Computer literacy in a Windows environment including email, word, power point, and excel.
  • Respect and understanding of The Salvation Army - its mission, culture, and values.

  • Non-violent Crisis Intervention training an asset.
  • Valid Driver’s License and Clean Driver’s Abstract an asset.

Responsibilities

  • Direct and monitor the work activities and resources of assigned care services to achieve quality, safe, and resident-centered care.
  • Participate in the development and implementation of plans, objectives, and strategies for care services which are aligned with overall organizational strategic directions.
  • Support complex case navigation together with the interdisciplinary team.
  • Evaluate the performance of the care services through agreed upon objectives and measures, and identify opportunities for quality improvement.
  • Assess, plan, implement and evaluate resident care collaboratively with the interdisciplinary team ensuring the delivery of resident-centered care. Approve and tour potential new intakes.
  • Provide direct support, advocacy, and communication to residents, families or guardians of the residents within the program.
  • Assist their Director in developing, recommending, implementing, and evaluating operational and clinical policies and procedures. Plan, develop, implement, and evaluate clinical care services/ plans in accordance with evidence-based practices, and applicable standards.
  • Ensure consistency in programs and practices in resident-centered care services.
  • Ensure that the resident care team complies with the organizational policies and procedures, continuing care health service standards, accommodation standards, relevant legislation regulations, accreditation standards, and regional health authority requirements.
  • Respond, investigate, and review concerns and complaints from residents, families, and staff.
  • Investigate and review all unusual incidents, critical incidents, and medication incidents.
  • Ensure that all intake/residential agreements are met and provide direct support to staff as they work with residents to be successful in meeting the expectations of residential agreements.
  • Ensure that resident admissions, transfers, and discharges are processed according to Policy and Procedures, contractual obligations centered on core value delivery.
  • Ensure that frequent and ongoing opportunities for residents to engage in positive social and community activities are offered.
  • Ensure concerns related to property, equipment, and environmental safety are reported and addressed, and that the nursing station and care areas are maintained in an organized, clean, and safe manner.
  • Responsible for scheduling and appropriating staff hours within the department.
  • Responsible for entering staff hours for payroll.
  • Maintain collective agreement compliance and effective ongoing working relationships with unions.
  • Monitor employee sick time and workers compensation utilizations to ensure effective resource use.
  • Maintain and build team culture and model teamwork and respectful workplace expectations; schedule and chair staff meetings.
  • Conduct regular performance appraisals and learning plans and address performance management in accordance with organizational policies.
  • Ensure all staff have current education and certification required for the job.
  • Ensure staffing complement maximizes resources to achieve optimal resident care.
  • Responsible for recruitment and performance management. discipline and termination of staff in consultation with Human Relations and the Director of Programs.
  • Constantly and directly review the skills and competencies of staff to adequately meet the requirement of the job.
  • Ensure that Occurrence Reports are completed and processed as per Policy & Procedure.
  • Ensure that concerns with property and equipment are reported and that all program areas are organized, clean, and tidy.
  • Ensure/ support the Director in the timely reporting of all required and ad hoc reporting requests.
  • Ensure that adequate program supplies are maintained.
  • Attend all meetings with interdisciplinary team.
  • Work collaboratively with the Director, case managers, physicians, pharmacy, other Salvation Army sites and programs.
  • Coordinate and lead family conferences and related meetings.
  • Oversee all incidents and concerns.
  • Responsible for stewarding the program budget and expenses.
  • Responsible to review expenditures and to monitor any variances in the department.
