About The Position

This is a full-time, awake overnight position in Red Oak, Ia. The hours are Sunday-Tuesday and every other Saturday 11pm-9am. The Supported Community Living Specialist (SCLS) is directly responsible to the Team Leader to which they are assigned and the Director of Residential Services. This is a part-time/full-time, Para-professional level position. The SCLS will teach, train, and assist individuals in developing the skills necessary to be as independent as possible when performing activities of daily living. This includes coordinating and training individuals to take care of their own health and medical needs, developing or maintaining skills for better community participation, assisting individuals in accessing community services, and assisting with participation in community activities. The role also involves assisting individuals in advocacy, preparing individualized diet plans, writing Individualized Intervention Plans, providing emergency/safety skills training, training in managing personal business and property, budgeting, and providing transportation for appointments and activities. The SCLS will orient new individuals, assess skills, provide input for progress reports and Person-Centered Plans (PCP), supervise goals and data collection, supervise finances for guidance in budgeting, provide a supportive and therapeutic atmosphere, supervise care and use of personal property, and assist with purchases. Additionally, the SCLS may train new staff, follow the assigned work schedule, ensure individuals receiving twenty-four-hour care are never left unsupervised, complete all records and documentation, attend staff meetings and in-service training, ensure residence meets cleanliness, safety, and health standards, stay informed of programming and administrative information, maintain strict confidentiality, document behaviors, and handle other assigned duties. The role also includes monitoring and documenting physician-ordered diets and assisting individuals with activities of daily living such as meal preparation, laundry, housekeeping, toileting, and bathing, and ensuring clients are safe from environmental hazards. Essential functions include providing quality care, transporting individuals, providing training in daily living skills, maintaining housekeeping standards, working with individuals who have behavioral problems (including verbal and physical aggression), providing a supportive and therapeutic atmosphere, administering medication after training, exercising sound judgment, working according to the assigned schedule (including remaining awake during overnight shifts), completing documentation, working independently, complying with HIPAA regulations, and assisting individuals with daily living activities. Employees are assigned a security clearance level based on their role and access to Protected Health Information (PHI).

Requirements

  • Individuals must not have or be a carrier of serious infectious or communicable disease.
  • 18 years of age.
  • High school diploma or GED.
  • Obtain and maintain a valid Iowa Chauffeur’s license.
  • Maintain acceptable driving record as determined by agency insurance carrier.
  • Successful completion of pre-employment physical, TB Tine, and drug screenings.
  • Complete 10-hour med-management class.
  • Must be willing to work with individuals who have a behavioral disorder, which may result in the display of aggressive outbursts of a physical nature.
  • Must be willing to clean any biological fluids/waste, such as blood, feces, urine, or vomit.
  • Must have the ability and desire to work with other employees as a team.
  • Must have good verbal and written communication skills.
  • Must be self-motivated.
  • Must not have been legally convicted of any type of assault, abuse, or bodily injury.
  • Must be able to travel from place to place, up to 70 miles.
  • Must be able to enter and exit resident homes that may or may not be accessible to everyone.
  • Must be able to walk up and down stairs with hands full to perform laundry duties, etc.
  • Must be able to work a full scheduled shift as assigned.
  • Must be able to carry necessary supplies up to 50 pounds to perform job duties.
  • Must be able to provide physical care necessary to residents when performing direct care duties.
  • Ability to physically assist an individual to regain his balance or to transfer from a sitting, standing or prone position.
  • Demonstrate ability to perform routine household tasks, such as vacuuming, mopping, and the use of cleaning agents, meal preparation, kitchen clean up, and clothing care.
  • Must be able to assist individual in a wheelchair to transfer to another chair, bed, toilet, or bath.
  • Must be able to assist someone in the shower or bath.
  • Must be able to make multiple trips from car to house carrying purchases such as groceries, etc.
  • Must be able to perform snow removal as much as necessary for individual and staff safety.

Nice To Haves

  • Previous similar job experience in teaching skills of cooking, cleaning, and personal hygiene.

Responsibilities

  • Teach, train, and assist individuals in developing skills for independence in activities of daily living.
  • Coordinate health and medical needs and train individuals to manage them.
  • Develop or maintain skills for better community participation and assist individuals in accessing community services.
  • Assist individuals with participation in at least one community activity each week if shift is during day/evening/weekend hours.
  • Assist individuals in advocacy procedures to ensure their rights and meet their needs, interests, and choices.
  • Assist in preparing individualized diet plans to meet nutritional needs.
  • Write Individualized Intervention Plans for each individual served.
  • Provide emergency/safety skills training.
  • Provide training in skills necessary to manage personal business and property.
  • Provide training in budgeting.
  • Provide transportation for doctor’s appointments, church, etc.
  • Provide training in daily living skills.
  • Orient new individuals in their home, assess their skills, and provide input to progress reports and Person-Centered Plans (PCP).
  • Supervise the individual’s goals and complete data collection.
  • Supervise, as necessary, the individual’s finances to give guidance in budgeting.
  • Provide a supportive and therapeutic atmosphere.
  • Supervise the individual’s care and use of personal property and assist with purchases.
  • Train new staff relative to duties, work schedules, agency policies, and state and/or federal regulatory policies and procedures.
  • Follow the assigned work schedule.
  • Never leave individuals receiving twenty-four-hour care unsupervised.
  • Complete all records and documentation using the proper forms.
  • Attend staff meetings and scheduled in-service training sessions.
  • Ensure that the individual’s residence meets cleanliness, safety, and health standards.
  • Check email each shift worked, the daily logbook, or other messages for programming and administrative information.
  • Keep all written and verbal information on each individual strictly confidential.
  • Complete documentation of behaviors or provide information for writing staff intervention plans.
  • Handle any other duties as assigned.
  • Monitor and document an individual’s physician ordered diet as needed.
  • Assist individuals receiving services with their activities of daily living, such as meal preparation, laundry, housekeeping, toileting, and bathing.
  • Ensure the clients are safe from all environmental hazards.
  • Provide quality care.
  • Administer medication and maintain accurate medication documentation, as needed, after being trained.
  • Exercise sound judgment when the situation requires immediate decision-making.
  • Work according to assigned work schedule; staff are expected to remain awake during overnight shifts.
  • Complete written and electronic documentation and record keeping according to agency requirements.
  • Work independently and ensure the welfare and safety of persons served in a residential setting.
  • Comply with state and federal regulations regarding HIPAA.
  • Participate and cooperate with agency investigations.
