Trillium Health Resources is seeking a Support Transition and Reentry (STAR) Specialist to join their team. The STAR Specialist will provide support to individuals enrolled in the STAR program, a specialty care team assisting adults with behavioral health or intellectual/developmental disabilities (IDD) who are exiting incarceration. Recipients are referred up to 90 days before release and receive care coordination pre- and post-release until they transition to ongoing Care Management services. The role involves educating members and guardians about navigating the criminal justice, behavioral, physical, and social services systems, as well as community-based services. The STAR Specialist will collaborate with the recipient's care team to facilitate transition linkages, including school, employment, housing coordination, community service opportunities, provider services, and other identified treatment supports. This position will also work with community resources to address reentry barriers such as access to insurance, disability benefits, transportation, food, clothing, and housing.
