About The Position

The Support Coordinator for Child & Adolescent Developmental Services ensures continuity of care for children and adolescents with intellectual and developmental disabilities by coordinating services and supporting informed, person-centered decisions. Key responsibilities include developing individualized, person-centered plans; connecting clients and families to appropriate providers and community resources; monitoring service delivery and progress toward outcomes; advocating for needs across systems; and supporting community integration across medical, educational, vocational, and recreational domains in alignment with discharge protocols.

Requirements

  • Bachelor’s degree in Human Services, such as Social Work, Psychology, or Counseling
  • One (1) year of experience in support coordination services with the assigned population
  • Must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
