About The Position

NLC Support is New Jersey’s #1 Support Coordination Agency, dedicated to serving adults and children with developmental disabilities. Our mission is to provide comprehensive support from start to finish. We help families identify and match the needs and wants of individuals with appropriate skills and expertise, connecting them with community providers and services. This is a hybrid position requiring field visits within Somerset and Middlesex Counties, with flexible work-from-home options available. The state of New Jersey requires Covid vaccination for this position. Candidates must also pass a criminal background and CARI check.

Requirements

One year of proven experience working with adults with developmental disabilities
Ability to take the initiative, work independently, make executive decisions when necessary, and meet deadlines
Excellent written and verbal communication skills
Reliable car
Flexibility to attend meetings when needed during the day
Flexibility to travel within Somerset and Middlesex counties
Must reside in the State of New Jersey
Must pass criminal background and CARI check
Covid vaccination required by the state of New Jersey

Responsibilities

Assessing service needs
Developing the New Jersey Individualized Service Plan (NJISP) for access to services
Liaison with clients, families, programs, and service providers
Establishing achievable goals, outcomes, and service eligibility
Development of a Person-Centered Planning Tool (PCPT)
Ongoing monthly monitoring of clients
Coordinate and facilitate team meetings off-site at either Residential and/or Day Programs to identify service needs
