Support Coordinator (QP) InReach has an exciting opportunity to join our team as a full-time Support Coordinator (Qualified Professional) in the Monroe area that is responsible to develop and coordinate Plans of Care for individuals with Intellectual/Developmental Disabilities. Will include meeting with IDD individuals and families to address needs and concerns; must also be willing to work cooperatively with community partners as well as staff from other InReach departments.

Requirements

4-year degree in the human services field
2 years of supervised direct experience working with the IDD population
Possess a valid driver's license
Access to reliable transportation 24-hours a day
Safe driving record.

Nice To Haves

1-2 years of management/supervisory experience preferred
Innovations Waiver and 1915i services experience preferred

Responsibilities

Develop and coordinate Plans of Care for individuals with Intellectual/Developmental Disabilities.
Meet with IDD individuals and families to address needs and concerns.
Work cooperatively with community partners as well as staff from other InReach departments.
Supervising Community Outreach staff in the implementation of supports to individuals served.
Monitoring, managing, and facilitating operations on a day-to-day basis.
Complete all documentation in a timely manner.
Implementation and review of customers Plan of Care.
Plan and oversee work schedule, approve payroll hours.
Provide one-on-one supervision as needed.
Share on-call service for emergencies.
