About The Position

Make a meaningful difference in the lives of adults with intellectual and developmental disabilities (IDD) as a Support Coordinator with the Hampton-Newport News Community Services Board (H-NNCSB). In this role, you will help individuals access the services, resources, and supports they need to live as independently and successfully as possible in the community.

Requirements

  • A Bachelor's degree in Human Services, such as Social Work, Psychology, Counseling, or a related field.
  • One year of experience providing case management or support coordination services to adults with developmental disabilities.
  • Must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.

Responsibilities

  • Develop and monitor individualized service plans based on each person's needs, strengths, preferences, and goals.
  • Coordinate medical, behavioral health, residential, employment, transportation, and community-based services.
  • Advocate for individuals and provide ongoing support, crisis intervention, and assistance with daily living needs.
  • Collaborate with individuals, families, healthcare professionals, and service providers to ensure appropriate and effective supports.
  • Promote independence, self-determination, community inclusion, and quality of life while maintaining accurate documentation.
